Kindly read the instructions given below very carefully:
 
ONLINE MODE :-

1.     Fill the form in Capital letters only.

2.     Online Registration will be done through the school website jldavps.com

3.     Payment will be made ONLINE.

4.     Keep your details of Debit Card/Credit Card/Net banking ready for online payment of Rs. 25/- as the registration amount

5.     In case of single parent, the entire information of the parent is to be repeated in both the columns.

6.     Fill up the form completely before taking a printout. In case of incomplete form/documents, the application will be deemed as rejected.

7.     Print out of the filled registration form, duly signed by the parent, must be submitted at school reception, along with self-attested photocopies of the supporting documents, on any working day between 8:30am – 2:30pm.

8.  If payment process is not completed successfully due to any reason, proceed again

OFFLINE MODE :-

Parents can also collect admission form at school reception from 8:00 am to 2:30 pm
Parents are requested to bring original documents at the time of verification

Required:

1. Proof of Date of Birth: Birth Certificate of child issued by MCD/NCMC/Muncipla Corporation.

2. Proof of Address (photo copy of parent's Aadhar card and Voter ID):
3. Aadhar card of Child:
  • Ration Card / Smart Card issued in the name of Parents (having name of child)
  • Domicile certificate of child or his / her parents
  • Voter I-Card (EPIC) of any of the parents.
  • Electricity bill / MTNL telephone bill / water bill in the name of any of the parents.
  • Passport issued by Govt. of India in the name of any of the parents.
  • Passbook of running bank account having the photograph of the parents, attested by the bank manage
  • Please note: Mobile phone and internet bills will not be acceptable.
  • Acknowledgment receipt of Adhar card / UID card will not be acceptable.
4. Proof of alumni of Jhabban Lal DAV Public School, Paschim Vihar (Passing Certificate) Class XII (Passing Certificate).

5. Proof of sibling: Latest fee bill / Report Card / Identity Card.
 
6. Proof of school leaving certificate and report card of the student admiting in class II to VIII.
 
Note:-   Adhar Card of child is mandatory for admission.
